APPLICATION
FORM FOR COPYRIGHT LICENCES
I/We,
the undersigned, apply for a licence from WORD OF LIFE INTERNATIONAL
which, subject to the terms and conditions set out herein, authorises
the multiple copying of protected copyright material. This licence
authorises the use of licence print copyright material used during
public religious worship meetings and weddings, funerals etc.

PURPOSE
OF THE LICENCES
These
licences enable the manufacture of copies of the Agency's members
owned or controlled copyrights limited by certain restrictions as
detailed in the terms and conditions attached.

ANNUAL RESTRICTED COPY LICENCE
The
fee scale is related to the average number of attendees per week for
churches or
number
of students for schools.
